Role OverviewAs a
Care Coordinator / Support Coordinator, you will be responsible for coordinating the support and care of a group of valued NDIS participants. You will assist participants in understanding and navigating their NDIS plans, identifying appropriate supports and services, and ensuring they receive the highest standard of care. The role involves developing and overseeing individualised support plans, coordinating care services, and ensuring that each participant’s goals are met through a tailored, person-centred approach.In addition, you will mentor and lead a team of support workers, ensuring that they have the skills to provide exceptional care and assistance to participants. You will also ensure compliance with NDIS regulations and contribute to the growth of the Townsville site through business development and marketing initiatives.
Key ResponsibilitiesSupport Coordination: Assist participants in navigating their NDIS plans, helping them access the services and supports they need to achieve their goals. This includes working with participants to identify and choose service providers, ensuring services are aligned with their funding and needs.
Participant Support: Coordinate and monitor the delivery of support services for a diverse group of NDIS participants, ensuring their needs are met through tailored, person-centred care.
Care Coordination: Develop, implement, and review individualised support plans, ensuring that each participant’s goals and preferences are prioritised in the service delivery. Monitor progress and adjust support plans as needed to achieve desired outcomes.
Team Leadership: Lead and mentor a team of support staff, providing guidance, training, and ongoing support to ensure high-quality, person-centred care is delivered consistently.
Staff Training & Development: Provide professional development opportunities for the team, ensuring all staff are equipped with the skills to meet NDIS requirements and deliver high-quality services.
NDIS Compliance: Ensure all services provided are fully compliant with NDIS standards and guidelines. Maintain accurate documentation for auditing and reporting purposes.
Participant & Family Engagement: Build and maintain strong, trusting relationships with participants and their families, acting as an advocate for their needs and ensuring their voices are heard.
Service Coordination: Collaborate with external healthcare providers and community services to ensure comprehensive support and seamless care delivery for participants.
Business Development & Marketing: Promote the services of the Townsville site by building relationships within the local community, identifying new opportunities, and engaging with prospective participants.
Quality Improvement: Regularly evaluate service delivery, contribute to continuous improvement initiatives, and ensure high standards of care are maintained.
Resource Management: Manage participant service budgets, ensuring resources are allocated effectively and efficiently.
Key Skills & ExperienceExperience in Support Coordination: Proven experience in supporting NDIS participants to implement and coordinate their NDIS plans, connecting them with appropriate service providers and ensuring continuity of care.Strong understanding of NDIS processes, regulations, and funding models.Proven experience in case management or coordinating care for individuals with disabilities.Excellent leadership, communication, and interpersonal skills, with the ability to engage effectively with participants, families, and staff.Ability to manage multiple priorities, with attention to detail and a strong problem-solving approach.Experience in business development or marketing within the disability services sector is a plus.A passion for supporting individuals with disabilities and improving their quality of life.
Qualifications- Relevant qualifications in Disability, Community Services, Social Work, or a related field.
- NDIS Worker Screening Check (or willingness to obtain).
- A valid driver’s licence and reliable vehicle for travel.
- First Aid and CPR certification.
- A current working knowledge of NDIS processes and service delivery.
