Role Description The Community Health Nurse is a full-time, on-site role based in Mount Gravatt, QLD. This role involves providing comprehensive community-based nursing care to NDIS participants, including health assessments, care planning, wound care, medication administration, and monitoring of chronic conditions. The nurse will conduct home visits and on-site support in SDA housing, deliver high-intensity care where required, and coordinate with multidisciplinary teams to ensure continuity of care. Daily responsibilities include client education on health promotion and disease prevention, documentation of clinical interventions, and collaboration with families and support coordinators to align care plans with individual goals. The role also requires adherence to clinical governance standards, participation in case conferences, and contribution to quality improvement initiatives within Kangaroo Care’s services.

Qualifications - Strong clinical foundation with skills in Nursing and Medicine appropriate to community and disability care settings.
- Demonstrated expertise in Community Health Nursing and Community Health, including health promotion and chronic disease management.
- Experience providing Home Care and community-based support, ideally within NDIS or similar frameworks.
- Current registration with AHPRA as a Registered Nurse and eligibility to work in Australia.
- Experience working with people with disability, complex medical needs, or high-intensity support requirements.
- Excellent communication, documentation, and interpersonal skills, with the ability to build trust with participants and families.
- Ability to work autonomously, manage time effectively, and respond calmly to changing clinical needs.
- Valid driver’s license and willingness to travel locally for home and community visits.
- Commitment to culturally safe, person-centered care and adherence to ethical and professional standards.
