Job Description:
Commencing 6th of July 2026 Permanent Full Time | Horsham, Dimboola, Edenhope or StawellYP2 - YP9 | $41.15 to $52.20 per hour About the RoleGrampians Health is currently seeking applications for the 2026 Transition to Practice (Graduate) Program with our Aged Care facilities. Join our caring and committed aged care teams across Horsham (Kurronjong Lodge & Wimmera Nursing home), Dimboola, Edenhope or Stawell. You'll deliver quality, resident-focused care while enjoying a regional lifestyle and opportunities for growth. Flexible hours available with the option to take on more as positions open. The successful applicants will be excellent communicators with a sound understanding/knowledge of ACFI, Aged Care Standards and Quality outcomes.
  • Opportunity to submit preferences of where you will be allocated prior to the conclusion of their Graduate transition program
  • comprehensive 4-day orientation program and a further 6 professional development days
  • Supernumerary time at the commencement of each rotation and regular Graduate debrief sessions
  • Develop self-confidence, critical thinking, decision-making, clinical skills, and clinical competence as part of the multifaceted role of the Registered Nurse
  • Facilitate practical and theoretical learning opportunities

Key Responsibilities
  • Work collaboratively with the Care Community Manager to facilitate and foster best practice standards in nursing care to residents and their families and carers.
  • Actively participate in the Graduate Nurse Program, including completion of all requirements outlined in Graduate Orientation.
  • Be responsible for planning, implementing, and evaluating evidence-based nursing care in collaboration with the multidisciplinary team to achieve optimal healthcare outcomes
  • Practices independently and interdependently assuming accountability and responsibility for their own actions and the delegation of care to other healthcare workers as required
  • Provide high-quality, comprehensive, and evidence-based patient-centred care whilst acting as a resource and role model to other staff

Skills & Experience
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to staff, patients/residents their families, and carers.
  • Maintains relationships with the health care team by communicating information, building rapport, and participating in team problem-solving.
  • Participates and engages in the annual performance review and professional development plan in accordance with the Grampians Health Staff Development Program

Visa Sponsorship Support (if required)
  • 407 Training Visa sponsorship available for eligible candidates with a potential pathway and employer support to permanent residency via TSS482 and ENS186.
  • Relocation support up to $12,000 for eligible applicants.
  • Accommodation support up to 6 weeks (subject to availability).
