About the RoleAs a Social Worker, you will play a key role in empowering individuals living with psychosocial disabilities to achieve their goals and improve their quality of life. Working within a multidisciplinary allied health team, you will provide both coordination and direct support to clients and their families.
Your responsibilities will include:- Delivering psychosocial (mental health) support and assessments
- Preparing plan review reports and biopsychosocial/family assessments
- Developing and implementing individual and budget plans
- Collaborating with allied health professionals to coordinate support services
- Supporting individuals and families experiencing social disadvantage
- Providing coordination of supports or specialised support coordination

Key Requirements:- Bachelor’s Degree in Social Work
- AASW Registration (or eligibility for registration)
- Strong verbal, written, and interpersonal communication skills
- Excellent time management and organisational skills
- Active listening skills and empathy
- Valid Working with Children Check, NDIS Worker Screening, Police Check (within 6 months), Driver’s Licence, car insurance, and a reliable vehicle
- Proficiency in Microsoft Office (Word, Excel, Outlook)
Desirable:- Experience with clients with dual diagnoses, forensic disability, or forensic mental health
- Knowledge and experience working within the NDIS framework
- Experience working with Culturally and Linguistically Diverse (CALD) communities
- Willingness to travel locally as required
