About the RoleAs a Social Worker, you will play a key role in empowering individuals living with psychosocial disabilities to achieve their goals and improve their quality of life. Working within a multidisciplinary allied health team, you will provide both coordination and direct support to clients and their families. Your responsibilities will include:

• Delivering psychosocial (mental health) support and assessments
• Preparing plan review reports and biopsychosocial/family assessments
• Developing and implementing individual and budget plans
• Collaborating with allied health professionals to coordinate support services
• Supporting individuals and families experiencing social disadvantage
• Providing coordination of supports or specialised support coordination

About YouYou’re an empathetic and proactive professional who thrives on helping others reach their potential. You enjoy working both independently and as part of a dedicated team. Key Requirements:

• Bachelor’s Degree in Social Work
• AASW Registration (or eligibility for registration)
• Strong verbal, written, and interpersonal communication skills
• Excellent time management and organisational skills
• Active listening skills and empathy
• Valid Working with Children Check, NDIS Worker Screening, Police Check (within 6 months), Driver’s Licence, car insurance, and a reliable vehicle
• Proficiency in Microsoft Office (Word, Excel, Outlook)

Desirable:

• Experience with clients with dual diagnoses, forensic disability, or forensic mental health
• Knowledge and experience working within the NDIS framework
• Experience working with Culturally and Linguistically Diverse (CALD) communities
• Willingness to travel locally as required
