Job Description:
Role: Licensed Physical Therapist providing personalized care to older adults in private homes and senior living communities.

Responsibilities

  • Offer one-on-one physical therapy sessions, including evaluations and developing tailored treatment plans.
  • Support patient mobility, independence, and overall wellness through evidence-based interventions.
  • Document care effectively using electronic medical records in compliance with standards.
  • Collaborate with an interdisciplinary team of health professionals.
  • Educate patients, caregivers, and staff on safety and aging-in-place strategies.
  • Utilize telehealth and virtual care to extend access and flexibility.

Qualifications & Skills

  • Valid PT license in the practice state or eligibility.
  • Degree from an accredited program.
  • Proficiency with EMR systems and basic computer skills.
  • Current CPR certification.

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to appointments, with necessary PPE use.
