Role: Licensed Physical Therapist providing personalized care to older adults in private homes and senior living communities.
Responsibilities
- Offer one-on-one physical therapy sessions, including evaluations and developing tailored treatment plans.
- Support patient mobility, independence, and overall wellness through evidence-based interventions.
- Document care effectively using electronic medical records in compliance with standards.
- Collaborate with an interdisciplinary team of health professionals.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Utilize telehealth and virtual care to extend access and flexibility.
Qualifications & Skills
- Valid PT license in the practice state or eligibility.
- Degree from an accredited program.
- Proficiency with EMR systems and basic computer skills.
- Current CPR certification.
Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to appointments, with necessary PPE use.
