A licensed Physical Therapist is responsible for delivering one-on-one physical therapy to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments within a flexible, patient-centered model.
Key Responsibilities
Perform evaluations, develop individualized care plans, and deliver therapy sessions supporting wellness and functional independence.
Document patient progress accurately in electronic medical records.
Collaborate with an interdisciplinary team including PTs, OTs, and SLPs.
Educate patients, caregivers, and staff on safety and aging in place.
Utilize telehealth and virtual care to enhance access and continuity.
Qualifications & Skills
Valid state licensure or eligibility, degree from an accredited program, proficiency in EMR and basic computer skills, and current CPR certification.
Physical capability to perform transfers, reposition patients, and perform therapeutic activities, including lifting and mobility tasks.
Must meet physical demands of in-home and community care.
