Job Description:
Bridges Care Services Pty Ltd is seeking Community Workers for our various homes around Sydney,Victoria and the Regional Area Nsw and Vic This is a full-time role.You'll be working with people with a range of mental and physical disabilities — supporting them to engage with their community, grow their independence, and live a life they value. About the Role:· Deliver community access support — from recreational outings to everyday appointments or assisting clients around their home.· Help clients build confidence navigating public spaces and social settings.· Work with people living with a range of mental health and physical disabilities.· Encourage life skills development, goal setting, and a positive daily routine.· Provide emotional support, companionship, and transport.
  • Assessing clients' needs and planning, developing and implementing educational, training and support programs

· Identifying specific challenges through client interviews and tailoring support to meet individual needs· Engage with families, welfare agencies, government organisations and private businesses to identify and address pressing community issues· Provide counselling and emotional support to help clients navigate personal difficulties· Preparing submissions for funding and resources, as well as reports for government bodies and other agencies About You:· Passionate about inclusion, empowerment, and community participation.· Experience in disability, mental health, or youth support is highly regarded.· Great interpersonal skills and professional boundaries.· Comfortable with personal support and flexible routines.· Confident in working with clients from diverse and socioeconomically disadvantaged backgrounds.· Driver's licence, reliable vehicle, and confident with technology. Essential Requirements:· First Aid & CPR· NDIS Worker Screening Check· Working with Children Check· NSW Police Check· National Police Check (or NSW Police Check for local residents)· Bachelor’s degree in community services, education, sociology, etc· 3 years relevant experienceJob Type: Full-timePay: $80,000.00 – $85,000.00 per yearBenefits:
