Job Overview


We are looking for a Community Practitioner to provide nursing care to patients in the community setting. Patients can be of all age groups. The intervention(s) can take place in the patients’ own home, a clinical setting, in a nursing home or holiday accommodation.


Post holder has delegated responsibility as a Care Co-ordinator for the planning, implementation and evaluation of nursing packages. They will have the appropriate skills and ability to work alone and be confident in making decisions without direct supervision.


You will have the ability to work within the structure of the Mental Health Measure and to carry out the duties as set out in the Community Care and Treatment Plan.


The post holder should be flexible, adaptable and aim to accommodate the needs of the service.


The successful applicant will be able to contribute to and improve the performance of the nursing team and the Community Mental Health Service (CMHS).


The post holder liaises with members of the Community Mental Health Services, the Local Primary Mental Health Support Service, Primary Health Care Team and associated agencies, voluntary groups ,patients, carers and families.


This post meets the definition of a lone worker.


Main duties of the job


Applicants will have significant experience working in an acute adult mental health setting and will require a sound knowledge of current issues pertaining to mental health, crisis intervention and home treatment.


Comprehensive Assessment, Risk Assessment and Management skills within the context of CTP are essential, as are excellent interpersonal and communication skills.


The ability to manage time and prioritise workload is fundamental to effective service delivery.


The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 12,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services to almost 400,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:


Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest.


Five community hospitals: Amman Valley Hospital and Llandovery Hospital in Carmarthenshire, Tregaron Hospital in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.


Two integrated care centres (Aberaeron and Cardigan, Ceredigion).


Community facilities, including:


48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes


Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
