Job Title: Care Coordinator
Overview:We seek a meticulous and detail-oriented Care Coordinator to join our team. The Care Coordinator will play a crucial role in coordinating the delivery of high-quality care services across Milton Keynes and Oxford, ensuring that Care Workers are matched to clients based on the level and type of care they need, along with, when possible, being in close proximity. Responsibilities:- Rostering and care planning
- Liaise with various departments to ensure smooth communication and collaboration.
- Track inventory levels and order materials as needed.
- Assist in quality control checks to maintain high standards of Care.
- Implement and maintain health and safety standards in the Care area.
- Provide administrative support to the Care team and clients.
- Ensure training records are updated and supervisions are scheduled at the correct time.
- Responsible for resourcing for new care packages and building on existing packages.
- Improving and maintaining the continuity of care for new and existing care packages.
- Maintain clear, accurate, appropriate and up-to-date digital records for all clients and care workers within your area.
- Responsible for cordially resolving customer queries promptly within your area to maintain a positive Company reputation as directed by your line manager.
- Ensure all operations are Company, CQC and Ofsted Compliant.
- Partake in Oncall duties.
- Support the team as directed by your line manager in delivering and supervising care services.
- Quality assurance duties.
- Medication records coordination.
- Contribute to the growth and service expansion targets set by the business.
Skills:- Proficient IT skills
- Excellent organisational skills with the ability to multitask effectively
- Strong communication skills to interact with team members, clients and stakeholders
- Attention to detail and accuracy in data management
- Ability to work well under pressure and meet tight deadlines
- Ideally, NVQ/QCF level 3 in Care.
- At least 1 year of experience working in home care.
- Knowledge of CQC and Ofsted compliance is essential.
- Ideally experienced in conducting care assessments and reviews and motivating and nurturing staff.
- Being flexible around working schedules, roles and responsibilities.
- Being a team player is highly crucial to this role.
- Must hold a full UK driving licence.
