Role: Licensed Physical Therapist providing in-home and senior living care with a focus on geriatric patients.
Responsibilities: Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document patient progress in EMR, collaborate with an interdisciplinary team, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, basic computer and EMR skills, and current CPR certification. Ability to meet physical demands including lifting, repositioning patients, and driving to care sites.
Key Duties: Provide one-on-one therapy in various settings, support patient wellness, and promote aging in place through education and care. Flexibility in scheduling and care delivery methods enhances patient access and outcomes.
