Job SummaryWe are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within assisted living, nursing home, or care home settings. Duties- Develop, implement, and regularly review personalised care plans tailored to individual resident needs, including dementia and Alzheimer's care.
- Oversee daily operations within the care environment, ensuring compliance with health and safety regulations.
- Supervise and support care staff, providing leadership and guidance to maintain high standards of service delivery.
- Administer medications in accordance with prescribed protocols, maintaining accurate records.
- Coordinate with healthcare professionals to ensure residents receive appropriate medical attention and support.
- Monitor residents' conditions closely, responding promptly to any changes or concerns.
- Organise training sessions for staff on topics such as medication administration, dementia care, and person-centred approaches.
- Foster a warm, respectful atmosphere that promotes independence and dignity for all residents.
- Manage administrative tasks related to staffing schedules, documentation, and compliance reporting.
Skills- Proven experience in assisted living or nursing home environments with a strong understanding of senior care needs.
- Extensive knowledge of dementia and Alzheimer's care techniques.
- Demonstrated leadership and management skills with supervising experience in a healthcare setting.
- Proficiency in developing and managing personalised care plans.
- Excellent organisational skills with the ability to multitask effectively under pressure.
- Strong communication skills to liaise confidently with residents, families, healthcare professionals, and team members.
