Job Summary
We are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within assisted living, nursing home, or care home settings. The successful candidate will be responsible for managing care plans, supervising staff, and ensuring the well-being of residents, particularly those requiring specialised dementia or Alzheimer's care. This role offers an opportunity to lead a compassionate team committed to enhancing residents' quality of life through personalised and professional care. Duties
  • Develop, implement, and regularly review personalised care plans tailored to individual resident needs, including dementia and Alzheimer's care.
  • Oversee daily operations within the care environment, ensuring compliance with health and safety regulations.
  • Supervise and support care staff, providing leadership and guidance to maintain high standards of service delivery.
  • Administer medications in accordance with prescribed protocols, maintaining accurate records.
  • Coordinate with healthcare professionals to ensure residents receive appropriate medical attention and support.
  • Monitor residents' conditions closely, responding promptly to any changes or concerns.
  • Organise training sessions for staff on topics such as medication administration, dementia care, and person-centred approaches.
  • Foster a warm, respectful atmosphere that promotes independence and dignity for all residents.
  • Manage administrative tasks related to staffing schedules, documentation, and compliance reporting.

Skills
  • Proven experience in assisted living or nursing home environments with a strong understanding of senior care needs.
  • Extensive knowledge of dementia and Alzheimer's care techniques.
  • Demonstrated leadership and management skills with supervising experience in a healthcare setting.
  • Proficiency in developing and managing personalised care plans.
  • Excellent organisational skills with the ability to multitask effectively under pressure.
  • Strong communication skills to liaise confidently with residents, families, healthcare professionals, and team members.
  • Knowledge of medication administration protocols and regulatory requirements within home care or care home settings.
