Essential Responsibilities:

• Conducting Start of Care (SOC) and Discharge OASIS assessments, developing personalized plans of care, providing skilled nursing care per the plan during home visits, monitoring patient condition, administering treatments, educating patients/caregivers, documenting care accurately and timely in the EMR and coordinating with the interdisciplinary team for quality care.
• Conduct comprehensive physical, psychological, and social assessments
• Develop and implement individualized care plans
• Document patient care using our electronic health record system
• Coordinating with the Physical Therapist, OT, ST, HHA for their respective visits
• Triaging calls (coordinating calls) with the administrative/scheduling staff and the patient to make sure the visit dates are confirmed according to the availability and convenience of the patient – this information is to be relayed periodically as it gets updated within the patient group in Home Health Notify.
• Coordinate care with physicians, therapists, and other healthcare providers
• Provide patient and family education
• Manage medication protocols
• Recommend adaptive equipment and tools for improved quality of life
• Optional: Provide specialized care including wound care, IV management, PICC Line, G-Tube, and Foley care

Qualifications:

• Current, unencumbered RN license
• Associate's or Bachelor's degree in Nursing
• Minimum of 2 year nursing experience (Required)
• Home health experience highly desired
• CPR Certification (Required)
• Valid driver's license and reliable transportation
• Strong assessment and documentation skills
• Ability to work independently and as part of a team
• Resilience to maintain composure under pressure
• Adaptability to work in diverse living conditions
