This position is essential in supporting our commitment to delivering compassionate and professional home health care. Responsibilities
  • Conduct thorough assessments of patients’ physical abilities, range of motion, strength, posture, and coordination.
  • Diagnose physical issues and develop individualized treatment plans based on the patient’s needs.
  • Design personalized exercise programs to improve movement and strength.
  • Recommend specific techniques or equipment to aid rehabilitation.
  • Set realistic goals with patients and track their progress.
  • Guide patients through exercises that improve mobility, balance, and strength.
  • Provide manual therapy, such as joint mobilization and soft tissue manipulation, to reduce pain and improve movement.
  • Instruct patients in posture correction and ergonomic techniques to avoid further injury.
  • Teach patients about their condition, treatment options, and the importance of following through with prescribed exercises.
  • Advise on lifestyle changes to support recovery, such as diet, posture, and activity modifications.

Qualifications
  • Valid PhysicalTherapist license in the applicable state(s).
  • Experience in a skilled nursing facility or home health care setting preferred.
  • Strong knowledge of medical terminology and case management practices.
  • Ability to develop effective treatment plans based on client assessments.
  • Excellent communication skills for interacting with clients, families, and healthcare professionals.
  • Understanding of physiology and its application in occupational therapy interventions.
  • Experience with early intervention strategies is a plus.
  • Flexibility to adapt to various client needs and environments.
