Job Description:
Care Forum Pty Ltd is currently seeking an experienced and motivated Occupational Therapist to join our growing allied health team delivering services to participants funded through the National Disability Insurance Scheme.This role focuses on supporting individuals with disability to develop the skills required for independence and participation in daily life. About the RoleAs an Occupational Therapist, you will provide a range of assessments, therapeutic supports and capacity-building interventions to help participants improve their independence, wellbeing and participation in everyday activities. Key responsibilities include:
  • Conduct functional capacity and occupational therapy assessments
  • Develop individualised therapy and support plans
  • Support participants in activities of daily living including self-care, mobility and community access
  • Assess and recommend assistive technology and environmental modifications
  • Deliver therapy across home, community and clinical settings
  • Provide education and support to families and support teams
  • Prepare reports for NDIS plan reviews and therapy outcomes
  • Monitor participant progress and adjust intervention plans
  • Work collaboratively with families, carers and multidisciplinary teams
  • Ensure compliance with NDIS standards and organisational policies

About YouWe are looking for a compassionate clinician who is committed to participant-centred practice and delivering meaningful outcomes. Essential requirements:
  • Bachelor or Master of Occupational Therapy
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • NDIS Worker Screening Check (or willing to apply)
  • Completion of the NDIS Worker Induction Module (or willing to complete)
  • Current Driver’s Licence
  • Knowledge of disability legislation and NDIS frameworks

Skills and Experience
  • Experience supporting people with disability across the lifespan
  • Ability to conduct assessments and write clinical reports
  • Experience recommending assistive technology and home modifications
  • Strong clinical reasoning and problem-solving skills
  • Ability to manage a caseload efficiently
  • Excellent written and verbal communication skills
  • Ability to work independently and within a multidisciplinary team
  • Experience working with NDIS participants (preferred)
