Job Description:
Seeking licensed Physical Therapists to deliver personalized, one-on-one care in patients' homes and senior living communities.

  • Provide evaluations, develop care plans, and administer evidence-based treatments to enhance mobility, independence, and overall well-being.
  • Utilize telehealth and virtual care to increase flexibility and patient access.
  • Document patient progress accurately within electronic medical records, collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
  • Educate patients, caregivers, and staff on safety and aging-in-place strategies.
  • Maintain physical requirements such as lifting, repositioning, and patient transfers, and drive to patient locations.

Qualifications Include

  • Valid PT license and degree from an accredited program.
  • Proficiency with computers and EMR systems.
  • Current CPR certification.
  • Ability to perform physical tasks related to patient care.
