Esse Care is a registered aged care provider delivering high-quality, person-centred care to older Australians in their own homes. Through the Support at Home program, we help our clients live independently, safely and well, on their terms, in the place they most want to be.

We’re growing, and we’re looking for a compassionate and experienced Case Manager/Care Partner to join our dedicated, close-knit team supporting older Australians across Melbourne.\The OpportunityThis is a dynamic, client-facing position where you’ll lead care coordination, manage individual support plans, and make a real impact on the lives of those you support. You’ll work within a close-knit, supportive team in a flat-structure environment, where you’re empowered to work autonomously while ensuring clients receive the right services at the right time.
Key Responsibilities
  • Build strong, respectful relationships with clients, families, support workers, and external providers
  • Coordinate tailored support services that reflect clients’ goals and health conditions
  • Conduct assessments, develop and review care plans for each client
  • Make decisions that prioritise client safety, wellbeing, and quality of life
  • Lead and supervise your team of Support Workers, ensuring service quality and continuity
  • Monitor and manage client budgets to ensure full and appropriate utilisation
  • Maintain compliance with current legislation, funding guidelines, and internal policies
  • Collaborate and communicate with the client’s network of supports
  • Document accurate and timely progress notes on client interactions, changes in condition, incidents, and follow-up actions
  • Review progress notes submitted by support workers, suppliers, and allied health professionals to ensure coordinated, high-quality care
  • Admission of new clients with intake assessments and meet and greets

About You
  • Proven experience in aged care, home care, community services or a health setting
  • Sound assessment, care planning and case management skills, with the judgement to coordinate safe, goal-aligned support
  • Confidence to work independently and manage competing priorities
  • Excellent communication, relationship-building and leadership skills
  • Working knowledge of My Aged Care and the Aged Care Quality Standards
  • Current National Police Check (or willingness to obtain)
  • Driver's licence, own vehicle, and willingness to travel across Melbourne
  • Bilingual or multilingual ability is a strong advantage, particularly Turkish, Arabic or other community languages spoken by our clients
  • A clinical background such as nursing or allied health is a bonus, but not required
