As a
Physical Therapist serving our
Veterans and Gold Star Families, you will be responsible for providing quality direct patient care, including screening, evaluation, development of initial and ongoing treatment plans, discharge planning, and case management.
DESCRIPTION - Assess residents to determine care needs
- Optimize residents' functionality and skills
- Complete all necessary documentation via Net Health
- Attend required meetings and in-services, including but not limited to family meetings, care plans, staffing, and utilization reviews
- In conjunction with the DOR, develop programs appropriate to the needs of facility residents across the continuum of care
- Remain compliant with infection control and environmental protocols while following OBRA Resident Rights Guidelines
- If requested, provide consultation and clinical supervision to PTAs
- Maintain state license and stay current on issues related to physical therapy (company-paid)
- Responsible for appropriate scheduling of residents
- Facilitate and complete family and facility staff in-services and education
- Assist the facility in meeting standards necessary to maintain state and federal certification
- Complete tasks deemed appropriate and assigned by the Director of Rehabilitation
- Serve Veterans and Gold Star Families with pride and commitment
REQUIRED SKILLS - Excellent communication skills, both written and verbal
- Ability to maintain professionalism in all situations
- Effective time management and the ability to manage a caseload
- Physical demands: required to perform a variety of duties, some of which fall into the moderate to heavy physical demand category
REQUIRED QUALIFICATIONS - Degree in Physical Therapy
- State licensure to practice in South Carolina
- Newly licensed Physical Therapists are welcome—we can support your needs
