We have an exciting opportunity for an enthusiastic and motivated individual to join the Learning Disability, Autism and Transition Social Work Team in Furness. We are a creative and supportive team working with young people and adults who have a learning disability and/or autism.

About the role As a social care worker, you will review and assess the needs of extraordinary individuals and their carers to ensure the development and delivery of appropriate person-centered, self-directed support. Using relevant legislative processes to qualify your practice.
The transition element of our team means we work with young people from the age of 14 years. Part of the social care worker role therefore, could mean reviewing some of those younger people and supporting our transition worker.
