Job Description:
Physical Therapist Role
Provide one-on-one physical therapy to older adults in private residences and senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatment and personalized care plans.
Responsibilities include conducting evaluations, developing treatment plans, delivering therapy sessions, and documenting within EMR systems. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists, and educate patients, caregivers, and staff to promote safety and aging in place.
This role offers autonomy, flexibility, and virtual care options, including telehealth, to enhance continuity and access. It emphasizes building strong patient relationships and contributing to community wellness.
Qualifications include a valid PT license, degree from an accredited program, computer proficiency, and CPR certification. Physical demands involve exerting medium effort, assisting with patient transfers, and driving to care locations.
