Role: Physical Therapist (PT) providing in-home, senior living, and virtual care to older adults.
Responsibilities
- Deliver one-on-one physical therapy in various settings such as private residences and senior living communities.
- Perform evaluations, develop personalized care plans, and provide evidence-based treatments to improve mobility, independence, and quality of life.
- Document patient progress accurately within electronic medical records.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Utilize telehealth and virtual care options to enhance access and flexibility.
Qualifications & Skills
- Valid Physical Therapy license or eligibility to obtain one.
- Degree from an accredited PT program.
- Proficiency in computer use and EMR systems.
- Current CPR certification.
Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and navigate community settings. Ability to drive to patient locations and wear PPE as needed.
