Job Description:
Role: Licensed Physical Therapist providing personalized in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy, promote mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and utilize telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from accredited program, proficiency in EMR and computers, CPR certification, ability to perform physical tasks such as lifting, repositioning, and driving to patient locations.
Key Duties: Support patient wellness, safety, and aging in place; create flexible treatment schedules; build strong patient relationships; and work within a collaborative, clinician-driven environment.
