The OpportunityThis is community nursing the way it should be: autonomous, varied and genuinely rewarding, without the rotating shifts of a hospital or residential facility. You'll visit clients across Melbourne, delivering clinical care, building real relationships, and making a tangible difference to people's day-to-day lives.We welcome both experienced community nurses and early-career RNs looking to move into in-home aged care. You'll be well supported by our team.
Key Responsibilities
- Deliver high-quality nursing care to clients in their homes, including health assessments, wound care, medication management and administration, continence support, and chronic disease management
- Develop, implement and review individualised care plans in line with the Strengthened Aged Care Quality Standards and Support at Home requirements
- Conduct clinical assessments, monitor client health, and escalate changes in condition appropriately
- Provide clinical guidance and supervision to support workers and other direct care staff
- Liaise with GPs, allied health professionals, families and carers to coordinate care
- Potential to take on some care management duties, including overseeing a client caseload, coordinating services, reviewing care plans, and acting as a key point of contact for clients and their families
- Maintain accurate, timely clinical documentation in line with regulatory and organisational requirements
- Contribute to a culture of continuous improvement, safety and clinical governance
About You
- Current registration as a Registered Nurse with AHPRA
- A genuine passion for supporting older people to live well at home
- Strong clinical assessment and care-planning skills
- Confident working autonomously and managing your own schedule
- Excellent communication and relationship-building skills
- Current driver's licence and access to a reliable, comprehensively insured vehicle (essential)
- Eligibility to work in Australia and willingness to complete aged care worker screening and police check
- Experience in community, aged care or home care nursing is highly regarded, though early-career RNs are encouraged to apply
