Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy sessions to improve mobility and independence, document within EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, basic computer and EMR proficiency, current CPR certification, and ability to meet physical demands including lifting, repositioning, and driving.
