Job Description:
Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy sessions to improve mobility and independence, document within EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills: Valid PT license, degree from an accredited program, basic computer and EMR proficiency, current CPR certification, and ability to meet physical demands including lifting, repositioning, and driving.
Key Duties: Provide one-on-one care in various settings, support patient wellness, promote safety, and extend care through virtual platforms. Flexibility in treatment scheduling and collaboration with healthcare professionals are emphasized.
