Responsibilities- Deliver personal care and support to clients in accordance with their individual care plans.
- Assist clients with daily living activities, including bathing, dressing, and meal preparation.
- Maintain accurate records of client progress and any changes in their condition.
- Communicate effectively with clients, families, and other healthcare professionals to ensure a coordinated approach to care.
- Drive clients to appointments or social activities as required, ensuring their safety during transport.
- Support clients in engaging with community resources and activities to enhance their quality of life.
- Utilise IT systems for documentation and communication purposes as needed.
Requirements- Previous experience in home care setting is preferred but not essential.
- A valid driving licence .
- Ability to understand and implement care plans effectively.
- Proficient in English, both spoken and written, with strong communication skills.
- Basic IT skills for record keeping and communication purposes.
- A compassionate nature with a genuine desire to help others.
- Ability to work independently as well as part of a team, demonstrating reliability and professionalism at all times.
