Physical Therapist

Seeking a licensed Physical Therapist to provide one-on-one in-home, senior living, and virtual care to older adults in various community settings.

Key responsibilities include: conducting evaluations, developing personalized treatment plans, delivering evidence-based therapy to enhance mobility and independence, documenting care in EMR, collaborating with an interdisciplinary team, and educating patients and caregivers on safety and aging-in-place strategies.
